New Consumer-Patient Family Education Approach
A health system approach can positively impact your patient and families.
When PFE isn’t effective it can lead to increases in length of hospital stays, delays in discharge, readmissions, and more visits to the ED (Berwick, Nolan, & Whittington, 2008).
As leaders and stewards of accountable care, you are urged to become champions focused on creating relevant and meaningful patient and family programs. As such it is imperative to invest time in order to evaluate the PFE programs, people and resources at your health system. Be willing to step back in order to objectively evaluate your organizations view of patient and families capacity for self-care management then be willing to advocate for change so the right kinds of resources are available.
