What we do

They start by understanding your current situation, then give insights to anticipate with likely decisions that need made. They help to create plans you and your family would use to navigate a caregiver’s journey. They become part of a support network, so you are not alone.

Vertie, our proprietary caregiver platform, is used for goal setting and plans, delivering education and extending your supportive community. It is also used for monitoring your own health in relation to the caregiver role.

No matter where you are on the journey, we can help! Our flexible resources match each phase of the caregiving process: before, early, middle or late. Each phase has unique insights and resources that are need to navigate the caregiving role.

We also want to know about the people taking care of you including the doctors, other disciplines , services being used and where are you seeking care.

    Negotiating health care needs starts with the recipient of care and their family discovering what matters most for health or life goals. We help to set goals and plans to achieve these healthcare journey goals and support learning that is needed to make decisions for them.

    To successfully navigate, people must have a general understanding of how healthcare is organized, provided, and coordinated. We help you find your way through the health system by understanding the basic organizational procedures and the communication networks.

    This stage focuses on helping you and your caregivers gain confidence to master self-care management knowledge and skills.

    Maintaining Health and Wellbeing

    This is a personal side of your health. Goals to maintain health and wellbeing range from regaining a specific level of functioning, preventing worsening of a condition, to a focus on a quality end of life.

  • Three important foundations are essential to successfully leading your caregiver journey.

    Health literacy is the ability to make sense of, understand and use healthcare related information such as words, expressions, and parts of the health system. It is also being able to make sense of numbers found in results, instructions and on prescriptions.

    Self-care management knowledge is the knowing the who, what, when, where and why of our health care. It includes medications, conditions, illnesses, people or teams, processes and systems. It is followed by mastering the skills or “hands on” part of doing care.

    Participation in online activities such as support groups and networks, formal classes, individual and group learning activities, music therapy, and meditation are important for health and wellbeing.

Flexible to offer several ways to learn

Virtual does not mean on your own. We offer a variety of ways to support learning based on what what’s best for you and your caregivers. Learn through special interest groups, live sessions or online courses. There is an option to work 1:1 with an experienced caregiving coach.

Our courses are provided by health professionals with direct experience in the caregiver role and experts in caring for seniors in the home health setting. Our health professionals include Registered Nurses, Certified Wound Care Specialists, Pharmacists and Physical, Occupational and Speech Therapists. Our team also includes healthcare professionals who specialize in children with chronic complex medical conditions.

Flexible to offer Vertie content in several languages

If you or a caregiver speak a language other than English, please know the Vertie platform, learning content and special interest groups are available in several languages. We are committed promoting health equity and providing access to digital health resources.

Flexible to keep others involved, Globally

Build a community that includes family, the caregiver(s) and others who are supporting care. Share goals, plans and important documents, such as a POLST form with them. Since Vertie is a HIPAA and GDPR compliant platform, a separate user account is required for each person.
